Dawn M. Woolard, 49, of West Brooklyn, passed away August 30, 2021 at OSF St. Paul Medical Center, Mendota after a battle with cancer.

Private family only services will be held Thursday, September 2, 2021 at the Merritt Funeral Home, Mendota with Rev. Susan Presley officiating. Cremation rites will be accorded after services and private burial will take place at a later date.

Dawn was born December 8, 1971 to John Harold and Joyce Lee (Root) O'Brien in Litchfield.

Dawn grew up in Hillsboro. She worked as a CNA and DSP for many years. Some of her hobbies were reading, playing PlayStation, bingo and the one thing she loved most was spending time with her kids and grandkids.

She is survived by her five children Joseph Cole of Mendota, Michael (Alison) O'Brien of West Brooklyn, Destiny (Jessie) McKay of Mendota, Haley O'Brien of Fairfield and Cheyenne Gruen of Mendota; 11 grandchildren Layla, Katrina, Zachary, Elsa, Jeffrey, Kinlee, Aiden, Serenity, Isabella, Braxton and Ethan; one sister Jenny (Rick) Schultz of Litchfield; one brother John (Michelle) O'Brien of Fairfield and many nieces and nephews.

She was preceded in death by her father Harold O'Brien in 1998, her mother Joyce Lee Chapman in 2017, her stepfathers Richard Norris in 1993 and Jeff Chapman in 2013.
